Verification of Clinical Hours of Experience must be a letter issued by your employer/direct supervisor, and must:
- Be dated within the last 3 months
- Be on formal letterhead
- State that you have completed at minimum 960 hours within the past 3 years
- State that those hours included “direct patient scoring and/or recording”
- Include the verifier’s full name, wet signature, email address and phone number
